You can now check your eligibility to the medical program of UJK by submitting this form. Please provide your personal data and attach the scan of your high school leaving certificate and transcript of records (grades). A member of the admission team will e-mail you with the feedback regarding your admission chances.

NOTE: 

• Some high school certificates include grades. In this case you can attach just one document
• You can submit only one document in each section, make sure your scan includes all the pages you want to submit
